|Waiver||All student's parents or guardians will be required to sign a waiver form (which will be supplied on the first day of the clinic or download a PDF file here) stating that you have no knowledge of any physical impairment that would affect this player from participating in this program. By signing the waiver, you agree that in case of accident while attending the clinic, you authorize the StrikeZone staff to act for you in securing medical treatment, and you release the StrikeZone, the StrikeZone director and staff of any liability. You also agree to abide by the Camper Code of Conduct. See the printed form for complete details and information.|
You will automatically receive confirmation, in the form of a receipt, via email. You MUST enter your
email address on the secure billing information page to receive this confirmation. If you do not receive an email
confirmation receipt, please call 703-823-0061 or send an email to email@example.com. Do not
mail the order form or order confirmation pages. You will NOT receive a second confirmation at a later date. The
charge posted on your credit card statement is your second confirmation.
|Payment Options||The StrikeZone Academy accepts Visa, MasterCard and Personal Checks.
(Personal checks can not be accepted online, only via mail or at the facility.)
100% Cash refunds will be issued only if a clinic/camp is cancelled by the StrikeZone due to lack of registration.
In this case credit vouchers or cash refunds will be issued (all service charges will be waived). If a clinic/camp
is cancelled due to inclement weather, the StrikeZone will issue a voucher for another class, guest pass or service
in the amount of the cancelled day. Credits are valid for a one year period from the date when issued. If you cancel prior to a clinic/camp, you must give a 14 day written notice
to receive a refund. A $25 service charge will be assessed on each camp refund. A $25 charge may be assessed for
any request to move to or switch to another clinic/camp--this will be waived if the original camp did not have enough participants to take place. The StrikeZone reserves the right to cancel clinic/camps
that do not meet minimum enrollment requirements due to lack of sufficient registration. All campers are subject to a code of conduct as cited on the clinics page. By clicking on
the "Continue to Secure Payment Form" button and finalizing the transaction you hereby agree to the terms
