The StrikeZone Academy Camp Registration Form
 Registration Instructions:
 
 1. 
Check box(es) to select desired clinics/camps (note that the "Date" listing is the first day of the clinic).
 
 
 2. 
Fill in the player information section at the bottom of the form - this is required for registration.
 
 
 3. 
Click the "Continue to Secure Payment Form" button at the bottom of the page to enter payment information and review your order on our secure webserver. You will be able to alter your order (and return to this page if necessary) before finalizing your transaction via the check-out page. VISA or MasterCard only accepted.
 
 
 4. 
You will receive a confirmation of your order via email. An information email will be sent out several days prior to the start date regarding what to bring, what to wear, etc..

TERMS & CONDITIONS
Waiver All student's parents or guardians will be required to sign a waiver form (which will be supplied on the first day of the clinic or download a PDF file here) stating that you have no knowledge of any physical impairment that would affect this player from participating in this program. By signing the waiver, you agree that in case of accident while attending the clinic, you authorize the StrikeZone staff to act for you in securing medical treatment, and you release the StrikeZone, the StrikeZone director and staff of any liability. You also agree to abide by the Camper Code of Conduct. See the printed form for complete details and information.
Confirmation

You will automatically receive confirmation, in the form of a receipt, via email. You MUST enter your email address on the secure billing information page to receive this confirmation. If you do not receive an email confirmation receipt, please call 703-823-0061 or send an email to info@thestrikezoneacademy.com. Do not mail the order form or order confirmation pages. You will NOT receive a second confirmation at a later date. The charge posted on your credit card statement is your second confirmation.
If you do not see the charge posted on your statement, your registration was not processed - please call or email us immediately.

If you do not receive an email update with camp information by the Wednesday before the camp start day, email immediately to double check your registration.

Payment Options The StrikeZone Academy accepts Visa, MasterCard and Personal Checks.
(Personal checks can not be accepted online, only via mail or at the facility.)
Cancellation/
Refund Policy

100% Cash refunds will be issued only if a clinic/camp is cancelled by the StrikeZone due to lack of registration. In this case credit vouchers or cash refunds will be issued (all service charges will be waived). If a clinic/camp is cancelled due to inclement weather, the StrikeZone will issue a voucher for another class, guest pass or service in the amount of the cancelled day. Credits are valid for a one year period from the date when issued. If you cancel prior to a clinic/camp, you must give a 14 day written notice to receive a refund. A $25 service charge will be assessed on each camp refund. A $25 charge may be assessed for any request to move to or switch to another clinic/camp--this will be waived if the original camp did not have enough participants to take place. The StrikeZone reserves the right to cancel clinic/camps that do not meet minimum enrollment requirements due to lack of sufficient registration. All campers are subject to a code of conduct as cited on the clinics page.
